HOURLY FEES & PAYMENT $90 Elementary In-Person | $70 Elementary Online $100 Secondary In-Person | $80 Secondary Online $125 Test Prep In-Person | $105 Test Prep Online o In-person sessions are subject to a minimum of 60 minutes. Online sessions may be booked in 15- minute increments. o If I extend a session because of late pick-up or mutual consent between both parties, the hourly rate applies. o I will receive an electronic invoice the first week of the month for all hours incurred during the previous month. o I may pay by credit or debit card via Stripe or by a check made out to: Focus Tutors. I understand that if payment is not received by the 14th of each month, the amount due on my account will be deducted from the credit card associated with my Teachworks account. CANCELATION o I may cancel an appointment up to 24 hours before a scheduled session by contacting the tutor directly. Missed sessions without 24-hour notice will be charged at the full rate. LATE ARRIVAL & CHANGE OF SCHEDULE o I understand that sessions are subject to the scheduled times. Those that begin late or end early due to the student and/or parent will be charged for the originally scheduled amount of time. o I understand that the tutor will wait fifteen minutes from the time of the scheduled session before leaving and considering that session canceled without sufficient notice . NO WARRANTIES o I am aware that Focus Tutors makes no promises or warranties with regards to the results of our services. SUPERVISION o I understand that it is required to have an adult present during in-home tutoring sessions. If no adult is present, then the session will be considered canceled without sufficient notice. WAIVER OF LIABILITY o I release Focus Tutors, LLC and its independent contractors from any liability of injury, loss, or damage to personal property associated with activities during tutoring sessions.
